Your own consent clauses and documents
Add your own checkboxes or PDF agreements to the intake welcome screen, and see what each patient accepted.

- As the owner, open the own consent clauses section in Account & Settings.
- Add a clause in your own wording, or upload one of your own PDFs, such as a treatment agreement, privacy statement or terms.
- Mark a clause as required if it must be ticked before the intake can start.
- Patients see your clauses on the intake welcome screen, under the consent MyDosha always collects. For a PDF they open it and accept with “I have read and accept”.
- Each patient's intake data shows what they accepted and when. For a document it opens exactly the version they accepted.
Good to know
Editing the wording or replacing a PDF creates a new version. MyDosha does not check or write the content of these documents; they remain yours.
